Laser Rental Form

This rental agreement is for the Cutting Edge Handheld Class IIIB Therapy Laser (Class IIIB 500mW), which operates at wavelengths of 650nm, 808nm, and 905nm, and includes safety glasses. The use of this equipment requires approval by a licensed veterinarian.

The Renter agrees to pay a rental fee of $99/week. A security deposit of $500 is required and will be refunded upon return of the Equipment in good condition. Should the renter not return the laser, $2000 replacement fee will be charged on the credit card provided. 

$99 we will be automatically run when the until is rented and at the end of each 7th day (168 hours) if rental is extended. Extension is not guaranteed, based on availability. No credit is given for early returns.

The Equipment shall only be used for the owner’s personal horse or dog for the condition discussed with the veterinarian.  The Renter agrees to comply with all applicable regulations, including laser safety standards. The renter agrees to follow safety measures such as using appropriate protective eyewear. 

The Renter assumes responsibility for any injuries or damages resulting from improper use or negligence.

The Equipment must be returned in the same condition as when it was delivered, excluding normal wear and tear. The Renter agrees to report any damage or malfunction immediately. The laser is protected by manufacturer warranty, yet the renter agrees to pay for any repairs resulting from misuse or negligence.

The Renter agrees to indemnify and hold harmless the Owner against any claims, damages, or liabilities arising from the Renter’s use of the Equipment
