I grant Petaluma Equine and Circle Oak Rehabilitation and its representatives and employees the right to take photographs of me and/or my horse(s) or dog(s), and to copyright, use and publish the same in print and/or electronically. 

I agree that Petaluma Equine and Circle Oak Rehabilitation may use such photographs of me and/or my horse(s) or dog(s) with or without my name and any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content. Such photography will be of the sole purpose for educating the community (or to that effect) or colleagues and shall be used in a manner consistent with good scientific or educational reporting practices.
